The Primary Care Provider (Family Med or Internal Med Physician) delivers preventive, chronic disease maintenance and episodic health care to patients of our Health Center within the identified scope of primary care practice. The Provider supports the mission and goals of our Health Center. Utilizes professional skills to diagnose and treat patients who present with both acute and chronic health problems. Ensures efficient and effective patient-centered, team-based care through collaborative teamwork, problem solving, and communication. Provides care consistent with medical best practices and the policies and procedures of the organization. Complies with standards of care and related laws and regulations including HIPAA and OSHA regulations. Protects the dignity, privacy and confidentiality of patients and their families.
Essential Functions
Adheres to the highest standards of medical practice, ethics, and professionalism at all times.
Performs history taking, physical examinations, diagnosis, and therapeutic treatment planning.
Orders diagnostic testing and required medications as indicated.
Includes basic principles of healthy living and health risk reduction as part of patient encounters.
Attends scheduled staff meetings as required.
Provides input into the development of medical policies and procedures.
Adheres to practice guidelines, policies, and procedures.
Aids in the assessment and development of treatment plans according to set standards and treatment regimens in collaboration with patient/parent/or significant other.
Implements care based on the treatment plan; evaluates progress toward desired outcomes.
Knowledgeable/compliant with our Health Centers financial and treatment eligibility guidelines.
Treats patients and co-workers with acceptance and respect for cultural differences.
Collaborates with Medical Director, staff, and other service agencies.
Makes referrals and consultations according to patient needs and organizational policies.
Addresses patient/family educational needs, plans and implements teaching, evaluates outcomes.
Utilizes resources appropriately.
Documents subjective, objective, assessment accurately, timely and completely into our Health Centers Electronic Medical Record.
Keeps problem and medication list current.
Meets productivity targets, demonstrating priority setting and organizational skills.
Provides age specific preventive and health maintenance care.
Provides age specific anticipatory guidance, teaching self-care and health promotion.
Consistently documents disclosures of protected health information.
Participates in quality improvement through peer review and other projects as necessary.
Virginia Community Healthcare Association (VCHA) is a non-profit membership organization that serves as the primary care association for Virginia’s thirty community health centers and look-alike health centers. Membership in VCHA is open to all Federally Qualified Health Centers (FQHCs) and Federally Qualified Health Center Look-Alikes operating delivery sites in Virginia. VCHA supports our health centers in their mission to provide access to health care regardless of geographic location or ability to pay.
